The number of injuries sustained by car occupants involving the head, thorax, spine, pelvis and the upper limbs have been reduced significantly during recent years. This is probably due to better safety features in the cars, especially the availability and usage of safety belts, airbags etc. Therefore one can notice clinically a relative increase in survivors of severe frontal crashes, but many of them have injuries to the lower extremities. To verify this, we analyzed the foot and ankle injuries of front seat passengers.
Der Anteil von Geländefahrzeugen an der Gesamtzahl von Personenkraftwagen betrug im Juli 1991 in den alten Bundesländern ca. 1,1 Prozent. Nach eigenen Erhebungen sind etwa 62 Prozent der Geländefahrzeuge mit einem Frontschutzbügel ausgestattet. Der Bundesminister für Verkehr hat die BASt beauftragt zu prüfen, ob solche Konstruktionen einen Einfluss auf die Verletzungen bei Kollisionen mit Fußgängern und Zweiradbenutzern haben. Dazu wurde das Datenmaterial der Unfallerhebung der Medizinischen Hochschule Hannover ausgewertet und die um fahrzeugtechnische Angaben ergänzten Unfalldaten aus Nordrhein-Westfalen betrachtet. Weiterhin wird von Komponententests berichtet, die den Anprall menschlicher Körperteile an das Fahrzeug simulieren. Mit den Ergebnissen dieser Untersuchung wurde ein Frontschutzbügel hergestellt, der weniger aggressiv gegenüber ungeschützten Verkehrsteilnehmern ist. Zur Quantifizierung der Gefährdung von Fußgängern wurden zwei gängige Geländefahrzeugtypen mit und ohne Frontschutzbügel gemäß dem Prüfvorschlag der EEVC-WG 10 zur Bestimmung der Fußgängerverträglichkeit von Pkw-Frontflächen getestet. Die Ergebnisse aus den Versuchsreihen wurden mit Ergebnissen aus Versuchen an normalen Pkw verglichen.Es kann festgestellt werden, dass bei einem Unfall mit Kopfanprall eines Kindes an ein mit Frontschutzbügel ausgestattetes Geländefahrzeug bei 20 km/h mit gleichen Kopfbelastungen zu rechnen ist, wie bei einem Unfall mit 30 km/h mit einem Geländefahrzeug ohne Frontschutzbügel, beziehungsweise mit 40 km/h mit einem normalen Pkw. Für den Hüftanprall eines Erwachsenen an die Haubenkante ist bei einer Fahrzeuggeschwindigkeit von 25 km/h bei einem Fahrzeug mit Frontschutzbügel mit gleichen Belastungen zu rechnen, wie bei einem Unfall mit einem Fahrzeug ohne Frontschutzbügel bei 40 km/h (Pkw oder Geländewagen). Für die Belastungen des Knies eines Erwachsenen lässt sich keine Verschlechterung durch montierte Frontschutzbügel ableiten.
Der Nutzen des Schutzhelmes für motorisierte Zweiradfahrer ist unumstritten. Die Grenzwerte für eine zulässige Kopfbeschleunigung bei der Prüfung des Dämpfungsverhaltens gemäß ECE-R 22 sind jedoch weiterhin in der Diskussion. In der vorliegenden Untersuchung wird deshalb die Kopfbelastung eines Dummys beim Pkw-Crash-Versuch mit den Belastungen des behelmten Prüfkopfes bei Fallversuchen gemäß ECE-R 22 verglichen. Bei den durchgeführten Fallversuchen besteht ein enger Zusammenhang zwischen HIC und maximaler Beschleunigung, der durch eine analytische Funktion beschrieben werden kann. Es kann gezeigt werden, dass die bei ECE-R 22 zulässige Kopfbeschleunigung von 300 g einem HIC von etwa 3400 entspricht. Zur Bewertung von Schutzhelmen mit verbesserten Stoßabsorptionseigenschaften erscheint es sinnvoll, wie anhand von Modellrechnungen gezeigt wird, den HIC als Schutzkriterium einzuführen. Bei einer Anprallgeschwindigkeit von 7 m/s kann ein HIC von 1500 als realistische Forderung angesehen werden, sofern bei der ECE-Regelung der sogenannte Zweitschlag entfällt. Bei den Pkw-Versuchen ist kein Zusammenhang zwischen HIC und maximaler Beschleunigung vorhanden. Hier ist der HIC als Verletzungskriterium etabliert, und das ist für den komplexen Verlauf des Signals der Kopfbeschleunigung sinnvoll. Eine Bewertung nach anderen Kopfschutzkriterien würde die Rangfolge der einzelnen Versuche ändern. Bei Helmversuchen auf den Stirnanprallpunkt B werden starke Rotationen des rückprallenden Systems Prüfkopf/Helm beobachtet. In Verbindung damit treten hohe Rotationsbeschleunigungen auf. Diese liegen, wie bei einem Teil der Helmversuche gemessen, zwischen 2,4 und 4,8 krad/s2.
Recently, EuroNCAP updated the upper legform test protocols. The main objective of this study is to establish the upper legform test in KIDAS (Korean In-depth Accident Study) taking into account domestic pedestrian accident data as well as anthropometric data to protect elderly pedestrians whose average height and weight is much smaller and lighter than other age groups, especially compared to Europeans. Therefore 230 cases of pedestrian accidents from KIDAS were investigated to explore the injury severity of body regions as well as age related injury patterns. Injuries of all body regions were examined, with a special focus on injuries of abdomen and pelvic area. On the other hand, in order to explore Korea's pedestrian accident environment, national police data and KIDAS (Korean In-depth Accident Study) data were compared. The results should be taken into account in future analyses and possible improvements, such as regulations and KNCAP test protocols, of the pedestrian safety policy in Korea.
Car occupants have a high level of mortality in road accidents, since passenger cars are the prevalent mode of transport. In 2013, car occupant fatalities accounted for 45% of all road accident fatalities in the EU. The objective of this research is the analysis of basic road safety parameters related to car occupants in the European countries over a period of 10 years (2004-2013), through the exploitation of the EU CARE database with disaggregate data on road accidents. Data from the EU Injury Database for the period 2005 - 2008 are used to identify injury patterns, and additional insight into accident causation for car occupants is offered through the use of in-depth accident data from the EC SafetyNet project Accident Causation System (SNACS). The results of the analysis allow for a better understanding of the car occupants' safety situation in Europe, thus providing useful support to decision makers working for the improvement of road safety level in Europe.
This study is aimed to investigate the correlations of impact conditions and dynamic responses with the injuries and injury severity of child pedestrians by accident reconstruction. For this purpose, the pedestrian accident cases were selected from Sweden and Germany with detailed information about injuries, accident cars, and accident environment. The selected accident cases were reconstructed using mathematical models of pedestrian and passenger car. The pedestrian models were generated based on the height, weight, and age of the pedestrian involved in accidents. The car models were built up based on the corresponding accident car. The impact speeds in simulations were defined based on the reported data. The calculated physical quantities were analyzed to find the correlation with injury outcomes registered in the accident database. The reconstruction approaches are discussed in terms of data collection, estimating vehicle impact speeds, pedestrian moving speeds and initial posture, secondary ground impact, validity of the mathematical models, as well as impact biomechanics.
Abstract: Für Kinder in Deutschland existiert im Pkw weiterhin ein höheres Risiko, im Straßenverkehr bei einem Unfall schwer verletzt oder getötet zu werden, als es für ungeschützte Verkehrsteilnehmer wie Fahrradfahrer oder Fußgänger besteht. Dies erscheint auf den ersten Blick nicht nachvollziehbar, da der Pkw und die vorgeschriebenen Kindersitze den Kindern eine hohe Sicherheit bieten müssten. Der vorliegende Projektbericht gibt Aufschluss über den aktuellen Stand der Sicherheit von Kindern im Pkw in Deutschland und zeigt Optimierungspotenzial auf. Die Schutzwirkung der Kindersitze hängt von mehreren Faktoren ab. Dabei stehen technische Aspekte, die hier detailliert untersucht wurden, im Vordergrund. Doch vor allem in den letzten Jahren zeigte sich immer wieder in Feldstudien, aber auch in der Unfallanalyse, dass Kinderschutzsysteme (KSS) oftmals nicht nach den Vorgaben installiert wurden. Wenn der Kindersitz und/oder das Kind nicht entsprechend der Bedienungsanleitung gesichert werden, kann sich das Schutzpotenzial der KSS reduzieren und gegen Null gehen. Im schlimmsten Fall stellt eine fehlerhafte Benutzung von KSS eine Gefahr sowohl für das Kind als auch für andere Pkw-Insassen dar. Die Unfallanalyse ergab keine Aussage darüber, ob verschiedene KSS-Modelle zu unterschiedlichen Verletzungsmustern führen, da die Unfallkonstellationen pro KSS-Modell zu verschieden waren. Des Weiteren wurde analysiert, inwieweit heutige Testverfahren mit diesen Konstellationen übereinstimmen. Das Ziel, mit wenigen Verfahren möglichst alle relevanten Unfälle abzubilden, wurde zum Teil ereicht. Zu viele Testverfahren mit unterschiedlichen Bewertungssystemen stellen heutzutage die Endverbraucher vor das Problem, dass nicht klar ist, welches KSS wirklich das beste ist. In der Realität hängt die Schutzwirkung zudem noch wesentlich von der KSS-Pkw-Kombination ab, die nur annähernd durch Versuche überprüft werden kann, da sich eine zu hohe Anzahl von Kombinationsmöglichkeiten ergibt. Anhand von mehr als 100 Versuchen wurde das Schutzpotenzial von verschiedenen aktuellen Kindersitzmodellen untersucht. Dabei wurden immer vermeintlich gute und schlechte KSS gegenübergestellt und unter gleichen Randbedingungen getestet. Ziel dieses Vorgehens war es, anhand der Messergebnisse Maßnahmen zu erkennen, die dem Schutz der Kinder dienlich sind. Zusammenfassend kann gesagt werden, dass prinzipiell für Kinder ein gutes Schutzniveau durch KSS besteht, was aber von KSS zu KSS verschieden sein kann. Die teureren KSS wiesen keine mechanische Zerstörung auf und überstanden alle Tests ohne ernsthafte Beschädigungen. Lediglich das oftmals als Dämpfungsmaterial eingesetzte Polystyrol nahm Energie auf und verformte sich wie vorgesehen plastisch. Bei den "Billig-Kindersitzen" ergab sich leider ein komplett anderes Bild. Sobald die Anforderungen über die der Gesetzgebung hinausgehen, ist immer weniger Schutz für die Kinder vorhanden. Besonders im Seitenaufprall offenbarten sich erhebliche Lücken in der Sicherheit. Dies reichte von mechanischem Versagen mancher Plastikteile bis hin zu konstruktiven Unzulänglichkeiten. Vor allem die Gurtführung muss deutlicher gekennzeichnet und mechanisch verstärkt werden. Trotz des hohen Schutzpotenzials einiger Kindersitze ist das Ende der Entwicklung noch nicht abzusehen. In allen Kindersitzklassen ist es prinzipiell möglich, die Belastungswerte weiter zu reduzieren. Dazu werden am Ende des Berichtes verschiedene Möglichkeiten aufgezeigt. Zum einen ist dies eine optimierte Babyschale, die durch eine Trägheitsbewegung gezielt Energie abbaut und das Baby in eine günstigere Position bringt, in der die Belastungen des Körpers reduziert sind. Zum anderen wird gezeigt, dass durch die feste Anbindung des KSS an den Pkw und die Reduzierung der Rotation um die Y-Achse die Belastungen für Kinder reduziert werden können. Größtes Entwicklungspotenzial bietet dabei das ISOFIX-System. Es ist bekannt, dass ISOFIX die Fehlbedienung des Kindersitzes und die Belastungen der Kinder deutlich reduzieren kann. Trotzdem ist die bisherige Marktdurchdringung von ISOFIX eher gering. Obwohl insgesamt ein positives Fazit gezogen werden kann, darf nicht aufgehört werden, die Kindersicherheit weiter zu verbessern. Denn nicht alle Kindersitze schützen heute gleich gut. Einige genügen lediglich den Gesetzesansprüchen und finden in vielen Fällen durch einen sehr geringen Verkaufspreis Zugang in die Pkw. Durch verschärfte Testbedingungen sollten diese Kindersitze abgeschafft und der Weg für neue und bessere Kinderschutzsysteme frei gemacht werden.
This study aimed at prediction of long bone fractures and assessment of lower extremity injury mechanisms in real world passenger car to pedestrian collision. For this purpose, two pedestrian accident cases with detail recorded lower limb injuries were reconstructed via combining MBS (Multi-body system) and FE (Finite element) methods. The code of PC Crash was used to determine the boundary conditions before collision, and then MBS models were used to reproduce the pedestrian kinematics and injuries during crash. Furthermore, a validated lower limb FE model was chosen to conduct reconstruction of injuries and prediction of long bone fracture via physical parameters of von Mises stress and bending moment. The injury outcomes from simulations were compared with hospital recorded injury data and the same long bone fracture patterns and positions can be observed. Moreover, the calculated long bone fracture tolerance corresponded to the outcome from cadaver tests. The result shows that FE model is capable to reproduce the dynamic injury process and is an effective tool to predict the risk of long bone fractures.
Powered Two Wheeler (Motorcycle) crashes are overrepresented in EU, England, and United States casualty statistics for both fatal and serious injuries. While regional geographic differences are evident for motorcycle size, type, and engine displacement, the casualty statistics consistently indicate significantly higher injury rates for all motorcycle riders when compared to car occupants. Accident analysis and reconstruction of these motorcycle crashes is a necessary process to gain further understanding of potential injury mitigation strategies. This paper focuses on the analysis of the rider post impact trajectory in the immediate moments following a crash. The rider and motorcycle, while loosely coupled by seating position leading up to a crash, quickly decouple as the crash forces develop. As a result, the rider moves relative to the motorcycle and relative to the collision partner. This movement, or trajectory, is primarily influenced by the type and configuration of the impact, the type and configuration of the motorcycle and collision partner, and the speeds involved. Understanding the rider's post impact trajectory will assist in the development of injury mitigation strategies. Both the free flight trajectory of the rider and the rider's trajectory as influenced by interaction with the motorcycle and collision partner are examined. Rider trajectories in full scale crash testing and real world motorcycle crashes are both studied and presented. The resulting physical evidence that can be observed by an accident analyst is discussed. The application of projectile motion physics is analyzed and the necessary input parameters, such as initial launch angle, are studied. This study will assist in understanding the post-impact dynamics of a motorcyclist, and will provide useful information to analysts evaluating real world crashes.
Every second counts when human lives are at stake. The increasingly safe design of vehicles presents rescuers with a serious challenge. Faced with high-strength steels and body reinforcements, even the most powerful cutters reach their limits. Therefore, incident commanders require information on the technical features and components installed, directly in the vehicle. Several tests have shown that such information helps to save valuable minutes. Therefore, a standardised A4 "rescue sheet" containing information on the location of cabin reinforcements, the tank, the battery, airbags, gas generators, control units etc. " and indicating adequate cutting points must be used throughout Europe. Hopefully, in a few years, the new eCall emergency call system will be in place everywhere in Europe. The system will transmit the relevant vehicle-specific data directly to the rescuers on-site. Until then, we need a simple and effective solution that saves lives.
Aim of the study was to evaluate the protective effect of bicycle helmets particularly considering injuries to the head and to the face. Accidents with the participation of bicyclists which occurred from 2000 to 2007 were chosen from GIDAS. We observed that injuries to the head and face were more severe in the group of non-helmeted riders. There seems to be no significant difference in injuries with AIS 3-6. Altogether 26 cyclists were killed. 2 of them wore a helmet (1% of helmeted cyclists), 24 did not (1% of non-helmeted cyclists). Only one killed rider (without helmet) did not suffer from polytrauma (only head injuries recorded). The findings seem to support the thesis of a preventive effect of the bicycle helmet, however the two groups are different in their characteristics related to riding speed. Necessarily we need a multivariate model to evaluate the effect of helmets.
Cycle helmets have continued to increase in popularity since their introduction half a century ago. Many studies indicate that overall, head injury can be significantly reduced by wearing them. This study was conducted using two distinct sets of real-world cycling collision data from Ireland, namely cases involving police collision reports and cases involving admission to a hospital emergency department. The analyses sought to simulate and analyse the protective performance of cycle helmets in such collision scenarios, by comparing the Head Injury Criterion score and peak head accelerations, both linear and angular. Cycle collisions were simulated using the specialised commercial software MADYMO. From the simulation results, these key metrics were compared between the same-scenario helmeted and unhelmeted cyclist models. Results showed that the inclusion of bicycle helmets reduced linear accelerations very significantly, but also increased angular accelerations significantly compared to unhelmeted situations. Given the modest protective performance of cycle helmets against angular accelerations, it is recommended that cycle helmet manufacturers and international test standards need to pay more attention to head angular accelerations.
Die Beurteilung der technischen und unfallrelevanten Eigenschaften der beiden Scheibenarten zeigt, dass die VSG-Scheibe gegenüber der ESG-Scheibe hinsichtlich der Verletzungs- und Unfallgefahr zwar überwiegend Vorteile aufweist; diese Vorteile sind jedoch vor allem vor dem Hintergrund steigender Gurtanlegequoten nicht als so entscheidend anzusehen, dass daraus die Begründung für ein Verbot der ESG-Scheibe abzuleiten wäre. Diese Feststellung wird insbesondere auch gestützt durch eine Nutzen-Kosten-Betrachtung, die zeigt, dass aus gesamtwirtschaftlicher Sicht bei einem generellen Ersatz der ESG-Scheibe durch die teurere VSG-Scheibe bei günstigen Annahmen der Nutzen unter den Kosten liegen würde.
Sedan type vehicles in which adult rear seat passengers were present and which were involved in frontal collisions were investigated, and the influence of unbelted rear seat passengers on the injuries of front seat occupants was studied. Unbelted rear seat passengers move forward during impact. It was observed that there were not only cases in which front seat occupants sustained injuries caused by direct contact with rear seat passengers, but also cases where front seat occupants received severe injuries due to additional force from rear seat passengers, either impacting directly or indirectly as a result of deformation of the front seat. Severe injuries of front seat occupants were observed in the latter cases. This research validates the importance of seat-belt use for rear seat passengers, not only to protect themselves but also to mitigate injuries of front seat occupants.
Detailed investigations and reconstructions of real accidents involving vulnerable road users

The aim of this research is to improve knowledge about vulnerable road users accidents and more specifically pedestrians or cyclists. This work has been based on a complete analysis of real accidents. From accidents chosen from an in-depth multidisciplinary investigation (psychology, technical, medical), we have tried to identify the configuration of the impact: car speed, pedestrian or cyclist orientations. Then, we have made a numerical modelling of the same configuration with a multibody software. In particular, we have reproduced the anthropometry of the victim and the front shape of the car. A first simulation has been performed on this starting configuration. Next, effects of some parameters such as car velocity or victim position at impact have been numerically studied in order to find the best correlations with all indications produced by the in-depth analysis. Finally, the retained configuration was close to the presumed real accident conditions because it reproduces in particular the same impact points on the car, the same injuries, and is according to the driver statement. This double approach associating an in-depth accident analysis and a numerical simulation has been applied on pedestrian-to-car and bicyclist-tocar accidents. It has allowed us to better understand the real kinematics of such impacts. Even if this method is based on a case to case study, it underlines which parameters are relevant on a vulnerable road user accident investigation and reconstruction.
Real world accident reconstruction with the Total Human Model for Safety (THUMS) in Pam-Crash

Further improvement of vehicle safety needs detailed analysis of real world accidents. According to GIDAS (German In-Depth Accident Study) most car to car front accidents occur at mid-crash severity. In this range thoracic injuries already occur. In this study a real world frontal crash with mid-crash severity out of the AARU database was reconstructed. The selected car to car accident was reconstructed by AARU by means of pc-crash software in order to get the initial dynamic accident conditions. These initial conditions were used to reconstruct the complete accident in more detail using FE models for the car structure and the occupants. Occupant simulations were performed with FE HIII-dummy models and the THUMS using Pam-Crash code. An initial THUMS validation was performed in order to verify the model-´s biofidelity by means of table-top test simulations. THUMS bone stiffness values were modified to match the real word occupant age. A comparison between driver and passenger restraint system loading was done, as well as an injury prediction comparison between the HIII-dummy model and THUMS response for both cases. Detailed comparison between the HIII-dummy models and THUMS regarding thoracic loading are discussed.
Im Jahr 2005 hat sich der positive Trend in der Entwicklung des Unfallgeschehens im Straßenverkehr in Deutschland fortgesetzt. Nach Schätzungen der Bundesanstalt für Straßenwesen wird die Zahl der im Straßenverkehr Getöteten um mehr als 7% niedriger ausfallen als 2004. Die Gesamtzahl aller polizeilich erfassten Straßenverkehrsunfälle wird leicht auf ca. 2,24 Mio Unfälle sinken (Vorjahreswert: (2,26 Mio.). Bei der Zahl der Unfälle mit Personenschaden wird im Vergleich zu 2004 (339.310 Unfälle) ein Rückgang um etwa 1% auf weniger als 337.000 erwartet. Die Zahl der bei diesen Unfällen verunglückten (verletzten und getöteten) Personen wird dabei um knapp 2% abnehmen und im Jahr 2005 bei etwa 438.000 liegen. Die Anzahl der Getöteten im Straßenverkehr wird insgesamt um mehr als 7% auf knapp 5.400 sinken (Vorjahreswert: 5.842). Während innerorts der Rückgang mit nur 1% auf unter 1.480 Getötete gering ausfällt, ist auf Bundesautobahnen mit einem Rückgang der Getötetenzahl von fast 5% auf weniger als 670 Getötete zu rechnen. Die Anzahl der Getöteten außerorts (ohne BAB) wird sich mit einem Rückgang von fast 10% im Vergleich der Ortslagen am deutlichsten verringern und von 3.664 Getöteten im Jahre 2004 auf etwa 3.350 im Jahre 2005 sinken. Die Veränderungen in den verschiedenen Verkehrsbeteiligungsarten unterscheiden sich ebenfalls deutlich. Die Gesamtzahl der getöteten Kraftfahrzeuginsassen (2004: 3.521) wird im Jahr 2005 um etwa 10% auf weniger als 3.200 Getötete deutlich sinken. Die Anzahl der getöteten Fahrer und Mitfahrer von Motorrädern (einschließlich Leichtkrafträder) sinkt gleichzeitig um etwa 2% auf annähernd 840 Getötete (2004: 858). Bei den Moped/Mofa-Fahrern (2004: 122 Getötete) ist nach dem deutlichen Rückgang der Zahl der Getöteten von 9% im Vorjahr in diesem Jahr keine Verbesserung zu erwarten. Die Anzahl der getöteten Moped/Mofa-Fahrer wird voraussichtlich bei etwa 120 Getöteten stagnieren. Bei den Radfahrern steht dem sehr deutlichen Rückgang im letzten Jahr (mit 22,9% von 616 auf 475 Getötete) in diesem Jahr ein Anstieg auf etwa 540 Getötete gegenüber. Bei der Anzahl getöteter Fußgänger ist dagegen ein Rückgang absehbar - um mehr als 10% auf weniger als 750 (2004: 838). Bei den jungen Verkehrsteilnehmern im Alter von 18 bis 24 Jahren ist mit einem Rückgang um fast 10% auf ungefähr 1.150 Getötete im Jahr 2005 (2004: 1.269) zu rechnen. Bei den Kindern bis unter 15 Jahren (2004: 153 Getötete) wird nach dem starken Rückgang im Jahr 2004 (26,4%) seit fünf Jahren erstmals wieder ein Anstieg zu verzeichnen sein " um etwa 20 getötete Kinder auf ungefähr 170 Getötete. Bei den Senioren (2004: 1.201 Getötete) ist für das Jahr 2005 im Vergleich zum Vorjahr (Rückgang um 9,6%) ein sehr geringer Rückgang auf etwas weniger als 1.200 Getötete zu erwarten. Der rückläufige Trend hält bei den Alkoholunfällen auch im Jahr 2005 an. Gegenüber dem Vorjahr sinkt die Zahl der alkoholbedingten Unfälle mit Personenschaden um etwa 350 (mehr als 1%) auf knapp 22.200 (2004: 22.548). Für die Fahrleistungen der Kraftfahrzeuge werden im Jahr 2005 kaum Veränderungen erwartet. Nach vorläufigen Schätzungen wird die Gesamtfahrleistung mit einer Veränderung von 697,1 Mrd. Fz-km im Jahr 2004 auf etwa 699 Mrd. Fz-km im Jahre 2005 nahezu stagnieren. Unter Berücksichtigung dieser Entwicklung wird sich die Getötetenrate im Jahr 2005 insgesamt um fast 8% verringern.
Der positive Trend in der Entwicklung des Unfallgeschehens im Straßenverkehr in Deutschland hat sich auch im Jahr 2006 fortgesetzt. Nach Schätzungen der Bundesanstalt für Straßenwesen ergeben sich voraussichtlich folgende Veränderungen gegenüber dem Vorjahr: Rückgang der tödlichen Unfälle von 5.361 auf annähernd 5.000 (- 6 %), Sinken der Gesamtzahl der polizeilich erfassten Unfälle von 2,25 Millionen auf rund 2,23 Millionen und Rückgang der Unfälle mit Personenschaden von 336.619 auf etwa 323.000 (- 4 %). Auf den Bundesautobahnen fällt der Rückgang der Zahl der Getöteten mit nur 1 % gering aus, außerorts (ohne Autobahnen) beträgt er etwa 8 %. Die Zahl der getöteten Kraftfahrzeuginsassen wird um etwa 4 % sinken (weniger als 3.000 Tote), die Zahl der getöteten Fahrer und Mitfahrer von Motorrädern geht um fast 10 % (auf weniger als 800 Tote) zurück. Bei den Radfahrern ist ein Rückgang von ebenfalls 10 % auf rund 500 Getötete zu erwarten, bei den Fußgängern ist ein leichter Rückgang von etwas mehr als 2 % absehbar. Bei den Kindern wird ein deutlicher Rückgang der Anzahl Getöteter um rund 20 % zu verzeichnen sein, ebenso bei den Jugendlichen (15 bis 17 Jahre). Bei den jungen Verkehrsteilnehmern im Alter von 18 bis 24 Jahren ist mit einem Rückgang um mehr als 10 % auf weniger als 1.000 Getötete zu rechnen, bei den Senioren wird der Rückgang etwa 4 % betragen. Bei den Alkoholunfällen hält der rückläufige Trend weiter an, gegenüber dem Vorjahr sinkt die Zahl der alkoholbedingten Unfälle mit Personenschaden um mehr als 10 % auf weniger als 20.000. Für die Fahrleistungen der Kraftfahrzeuge wird im Jahr 2006 ein leichter Rückgang auf allen Straßenklassen erwartet, die Gesamtfahrleistung wird schätzungsweise 681 Milliarden Fahrzeug-Kilometer betragen (- 0,5 %). Unter Berücksichtigung dieser Entwicklung wird sich die Getötetenrate im Jahr 2006 insgesamt um mehr als 5 % verringern.
The improvement of passive car security devices led to a reduction of injuries, especially of the head, the neck and the torso mainly due to the airbag function. The passenger's foot and ankle could not profit from this development. Some investigators even reported a progression of leg injuries (1). In this study, we investigated a current collective of patients with foot and ankle fractures or severe soft tissue injuries in relation with defined crash parameters. Special interest was paid to the car's footwell.
Nowadays airbags are part of the standard equipment in almost all new cars. While airbags are saving an increasing number of people from severe injuries and death in moderate and high speed crashes, they do not completely prevent dashboard injuries. The most common mechanism in dashboard injuries is a posteriorly directed force to the proximal tibia with the knee flexed. This may occur during a motor vehicle frontal impact accident when a knee of the driver or the front-seat passenger strikes the dashboard. The posterior force can be combined with a abducting or rotational force leading to concomitant lateral or posterolateral injury. Car and airbag manufacturers therefore develop special inflatable systems to reduce the impact force in dashboard injuries. Every new inflatable system, however, has to be evaluated in out of position situations in which the system might cause injuries to certain body areas. Therefore, we investigated a new kneebag system in different critical seating positions of post mortem test subjects (PMTS). The tested knee airbag module is a folded airbag (18 litre volume) which is installed below the lower section of the instrument panel of a passenger car. Using four PMTS (2 male, 2 female, age 36"67) the following positions were tested: normal seating position, knee flexed >90 degrees and knee flexed <60 degrees in static deployment tests with direct contact. In addition a dynamic test (48.8kph, AAMA-pulse) was carried out with the PMTS belted in a normal seating position. The inflation phase and the impact of the system on the knee/lower leg were analysed by high speed videos. After the test the lower legs of the PMTS were examined by Xray and autopsy. All soft tissue injuries and bone fractures were recorded. All the tests could be evaluated. Except some superficial skin lesions in the impact area no fracture of the bones around the knee and no knee ligament and tendon injuries were observed. Neither video analysis nor autopsy of the PMTS showed any critical contact injuries caused by the inflation process of the bag. Therefore, it can be concluded that in the tested seating positions which are the most critical for the knee area the knee bag system is safe.
The primary goal of this investigation was to determine the relative risk of traffic accidents in students. In a two year period, a survey amongst 2,325 students was carried out, and 3,645 injuries sustained by students treated at our hospital were analyzed. Moped-riding in adolescents were associated with a 23.75-fold increased risk for injury as compared to biking. Children who ride bicycles have a 2.2-fold increased risk for an injury sustained by traffic accidents compared to pedestrians. None of 50 injured bicycle riders with helmet had an AIS for head injuries of more than 2. 24 of 233 injured bicycle drivers without helmet had an AIS for head injuries of more than 2. The use of a protective helmet significantly reduced the severity of head injuries. The level of awareness towards danger and a history of previous accidents correlate with the likelihood of future accidents. Due to the severity of traffic accidents, more adequate prevention measures (wearing of bicycle helmets and better education for moped riders) are urgently needed.
To date, the Trauma Registry (TraumaRegister DGU-® contains data of approximately 100.000 severely injured patients, 65% of which suffered from a road traffic crash. Thus, it is the world's largest data base for severely injured patients. The article describes the development of the registry and explains how it was rolled out over Germany using the established structure of the German Trauma Network (TraumaNetzwerk DGU-®). In addition, this article presents three typical use cases from the fields of quality management, policy making and system-wide interventions, clinical research and injury prevention. In conclusion, the TraumaRegister DGU-® is a well-established tool for various purposes related to the control and reduction of the burden of road injury. Its ongoing expansion to other countries will support the goal of international benchmarking of hospitals and trauma systems.
Die vorliegende Untersuchung gibt einen zusammenfassenden Überblick über die Leistungen des öffentlichen Rettungsdienstes in der Bundesrepublik Deutschland im Zeitraum 2004/05. Die wesentlichsten Ergebnisse des Forschungsprojektes sind: Bundesweit werden im öffentlichen Rettungsdienst im Zeitraum 2004/05 jährlich rund 10,2 Mio. Einsätze mit insgesamt 12,1 Mio. Einsatzfahrten durchgeführt. Die Einsatzrate beträgt rund 123 Einsätze pro 1.000 Einwohner und Jahr. An einem mittleren Werktag gehen bundesweit rund 30.900 Hilfeersuchen in den Rettungsleitstellen ein. Am Wochenende sinkt die Zahl der eingehenden Hilfeersuchen auf rund 21.600 an einem mittleren Samstag und auf rund 18.900 an einem mittleren Sonntag. 46 % des Einsatzaufkommens werden vom Leitstellenpersonal als Notfall eingestuft, 56 % entfallen auf die Kategorie Krankentransport. Praktisch die Hälfte aller Notfalleinsätze werden unter Hinzunahme eines Notarztes durchgeführt (Notarzteinsatz). Zwei Drittel der Notfälle zu Verkehrsunfällen (41 %) werden von einem Notarzt bedient. Rund 85 % des Notarzteinsatzaufkommens werden im Rendezvous-System und 15 % im Stationssystem bedient. Damit besteht auch im Zeitraum 2004/05 eine deutliche Dominanz des Rendezvous-Systems gegenüber dem Stationssystem. Rund jeder 19. Notfalleinsatz gilt einem Verkehrsunfall, was bundesweit rund 250.000 Einsätzen entspricht. Die Verteilung der übrigen Einsatzanlässe bei Notfällen mit und ohne Notarztbeteiligung beträgt: Internistischer Notfall 44 %, Sonstiger Notfall (z. B. Verbrechen, Suizid, dringende Blut- und Organtransporte) 37 %, Sonstiger Unfall (z.B. Haus-, Schul- und Sportunfall) 12 % und Arbeitsunfall 1 %. Die Verteilung der Rettungsmitteltypen am bundesweiten Einsatzfahrtaufkommen im Zeitraum 2004/05 beträgt: RTW 51 %, KTW 30 %, NEF 7 %, NAW 1 % und RTH/ITH unter 1 %. Beim Einsatzfahrtaufkommen werden rund die Hälfte der Einsatzfahrten mit Sonderrechten auf Anfahrt durchgeführt. Dies entspricht bundesweit jährlich 5,7 Mio. Einsatzfahrten unter Sonderrechten auf Anfahrt. Das Einsatzfahrtaufkommen weist im Bundesgebiet 2004/05 einen Fehlfahrtanteil von 8 % auf. Bundesweit sind dies jährlich über 996.000 Fehlfahrten. Mit zunehmender Einsatzdringlichkeit steigt der Fehlfahrtanteil. Für das Notfallaufkommen sind bundesweit rund 723.000 Fehlfahrten zu erwarten, für das KTP-Aufkommen rund 278.000. Die Dispositions- und Alarmierungszeit bei Einsatzfahrten mit Sonderrechten auf Anfahrt beträgt im Mittel 1,5 Minuten. Bei Einsatzfahrten ohne Sonderrechte auf Anfahrt beträgt die Dispositions- und Alarmierungszeit im Mittel 12,4 Minuten. Bei Einsätzen mit Sonderrechten auf Anfahrt errechnet sich nach dem zuerst eingetroffenen Rettungsmittel am Einsatzort eine mittlere Hilfsfrist von 8,1 Minuten, wobei 95 % der Notfälle innerhalb von 16,4 Minuten mit einem Rettungsmittel bedient werden. Die mittlere Hilfsfrist zu Verkehrsunfällen beträgt an Straßen innerorts am Tag 7,4 Minuten und in der Nacht 8,5 Minuten, an Straßen außerorts am Tag 8,6 Minuten und in der Nacht 10,9 Minuten. Die Unterscheidung der Einsatzzeit nach Notfällen und Krankentransporten unter zwei Stunden ergibt eine mittlere Einsatzzeit von 49 Minuten für Einsatzfahrten mit Sonderrechten auf Anfahrt und 52 Minuten für Einsatzfahrten ohne Sonderrechte auf Anfahrt. Die Transportzeit bei Einsatzfahrten mit Sonderrechten auf Anfahrt beträgt im Mittel 12,3 Minuten. Bei Einsatzfahrten ohne Sonderrechte auf Anfahrt liegt die Transportzeit im Mittel bei 15,9 Minuten. Die Verweilzeit am Transportziel/Wiederherstellungszeit bei Einsatzfahrten mit Sonderrechten auf Anfahrt beträgt im Mittel 18,3 Minuten, während bei Einsatzfahrten ohne Sonderrechte auf Anfahrt der Vergleichswert im Mittel bei 15,0 Minuten liegt.

Die wesentlichsten Ergebnisse des Forschungsprojektes sind: " Bundesweit werden im öffentlichen Rettungsdienst im Zeitraum 2008/09 jährlich rund 11,4 Mio. Einsätze mit insgesamt 14,2 Mio. Einsatzfahrten durchgeführt. Die Einsatzrate beträgt rund 143 Einsätze pro 1.000 Einwohner und Jahr. " An einem mittleren Werktag gehen bundesweit rund 35.000 rettungsdienstliche Hilfeersuchen in den Rettungsleitstellen ein. Am Wochenende sinkt die Zahl der eingehenden Hilfeersuchen auf rund 26.000 an einem mittleren Samstag und auf rund 24.000 an einem mittleren Sonntag. " 49 % des Einsatzaufkommens werden vom Leitstellenpersonal als Notfall eingestuft, 51 % entfallen auf die Kategorie Krankentransport. " Praktisch die Hälfte aller Notfalleinsätze werden unter Hinzunahme eines Notarztes durchgeführt (Notarzteinsatz). Ein Drittel der Notfälle zu Verkehrsunfaellen (32 %) wird von einem Notarzt bedient. " Das Rendezvous-System hat sich mit einem Anteil von 99,1 % gegenüber dem Stationssystem bundesweit durchgesetzt. " Rund jeder 17. Notfalleinsatz gilt einem Verkehrsunfall, was bundesweit rund 336.000 Einsätzen entspricht. Die Verteilung der übrigen Einsatzanlässe bei Notfällen mit und ohne Notarztbeteiligung beträgt: Internistischer Notfall 46 %, Sonstiger Notfall (z. B. Verbrechen, Suizid, dringende Blut- und Organtransporte) 37 %, Sonstiger Unfall (z.B. Haus-, Schul- und Sportunfall) 11 % und Arbeitsunfall unter 1 %. " Die Verteilung der Rettungsmitteltypen am bundesweiten Einsatzfahrtaufkommen im Zeitraum 2008/09 betraegt: RTW 52 %, KTW 29 %, NEF 18 %, NAW und RTH/ITH unter 1 %. " Beim Einsatzfahrtaufkommen werden rund die Haelfte der Einsatzfahrten mit Sonderrechten auf Anfahrt durchgeführt. Dies entspricht bundesweit jährlich 7,2 Mio. Einsatzfahrten unter Sonderrechten auf Anfahrt. " Das Einsatzfahrtaufkommen weist im Bundesgebiet 2008/09 einen Fehlfahrtanteil von unter 6 % auf. Bundesweit sind dies jährlich rund 808.000 Fehlfahrten. " Die Dispositions- und Alarmierungszeit bei Einsatzfahrten mit Sonderrechten auf Anfahrt beträgt im Mittel 2,1 Minuten. Bei Einsatzfahrten ohne Sonderrechte auf Anfahrt liegt die Dispositionsund Alarmierungszeit im Mittel bei 14,3 Minuten. " Bei Einsätzen mit Sonderrechten auf Anfahrt errechnet sich nach dem zuerst eingetroffenen Rettungsmittel am Einsatzort eine mittlere Hilfsfrist von 8,7 Minuten, wobei 95 % der Notfälle innerhalb von 16,7 Minuten mit einem Rettungsmittel bedient werden. " Die mittlere Hilfsfrist zu Verkehrsunfällen beträgt an Straßen innerorts am Tag 8,8 Minuten und in der Nacht 9,4 Minuten, an Straßen außerorts am Tag 10,4 Minuten und in der Nacht 10,9 Minuten. " Die Unterscheidung der Einsatzzeit nach Notfällen und Krankentransporten unter zwei Stunden ergibt eine mittlere Einsatzzeit von 51 Minuten für Einsatzfahrten mit Sonderrechten auf Anfahrt und 53 Minuten für Einsatzfahrten ohne Sonderrechte auf Anfahrt. " Die Transportzeit bei Einsatzfahrten mit Sonderrechten auf Anfahrt beträgt im Mittel 12,3 Minuten. Bei Einsatzfahrten ohne Sonderrechte auf Anfahrt liegt die Transportzeit im Mittel bei 16,1 Minuten. " Die Verweilzeit am Transportziel/Wiederherstellungszeit bei Einsatzfahrten mit Sonderrechten auf Anfahrt beträgt im Mittel 20,1 Minuten, während bei Einsatzfahrten ohne Sonderrechte auf Anfahrt der Vergleichswert im Mittel bei 15,9 Minuten liegt. " Die weiteren Ergebnisse der Pilotstudie zur Machbarkeit einer Datenerhebung und -analyse über die Ermittlung der Verletzungsschwere bei Verkehrsunfallopfern zeigen, dass die Analyse mittels Daten sowohl zur Rückmeldezahl in Hessen als auch mit Hilfe von DIVI-Notarztprotokollen möglich ist. Dabei ist eine Klassifikation mit Hilfe von Geodaten EDV-gestuetzt umsetzbar, um eine vergleichende Auswertebasis zu bilden.
Die wesentlichsten Ergebnisse des Forschungsprojektes sind: - Bundesweit werden im öffentlichen Rettungsdienst im Zeitraum 2012/13 jährlich rund 12,0 Mio. Einsätze mit insgesamt 14,3 Mio. Einsatzfahrten durchgeführt. Die Einsatzrate beträgt rund 147 Einsätze pro 1.000 Einwohner und Jahr. - 52 % des Einsatzaufkommens werden vom Leitstellenpersonal als Notfall eingestuft, 48 % entfallen auf die Kategorie Krankentransport. - Über zwei Fünftel aller Notfalleinsätze werden unter Hinzunahme eines Notarztes durchgeführt (Notarzteinsatz). Fast ein Drittel der Notfälle zu Verkehrsunfällen (29 %) wird von einem Notarzt bedient. - Rund 3,5 % der Notfalleinsätze gelten einem Verkehrsunfall, was bundesweit rund 208.000 Einsätzen entspricht. Die Verteilung der übrigen Einsatzanlässe bei Notfällen mit und ohne Notarztbeteiligung beträgt: Sonstiger Notfall 51 %, Internistischer Notfall 34 %, Sonstiger Unfall (z.B. Haus-, Schul- und Sportunfall) 11 % und Arbeitsunfall unter 1 %. - Die Verteilung der Rettungsmitteltypen am bundesweiten Einsatzfahrtaufkommen im Zeitraum 2012/13 beträgt: RTW 57 %, KTW 24 %, NEF 18 %, NAW und RTH/ITH unter 1 %. - Beim Einsatzfahrtaufkommen werden rund die Hälfte der Einsatzfahrten mit Sonderrechten bei der Anfahrt durchgeführt. Dies entspricht bundesweit jährlich 8,4 Mio. Einsatzfahrten unter Sonderrechten bei der Anfahrt. - Die Dispositions- und Alarmierungszeit bei Einsatzfahrten mit Sonderrechten bei der Anfahrt beträgt im Mittel 2,5 Minuten. Bei Einsatzfahrten ohne Sonderrechte bei der Anfahrt liegt die Dispositions- und Alarmierungszeit im Mittel bei 14,5 Minuten. - Bei Einsätzen mit Sonderrechten bei der Anfahrt errechnet sich nach dem zuerst eingetroffenen Rettungsmittel am Einsatzort eine mittlere Hilfsfrist von 8,4 Minuten, wobei 95 % der Notfälle innerhalb von 16,9 Minuten mit einem Rettungsmittel bedient werden. - Die Unterscheidung der Einsatzzeit nach Notfällen und Krankentransporten unter zwei Stunden ergibt eine mittlere Einsatzzeit von 52 Minuten für Einsatzfahrten mit Sonderrechten bei der Anfahrt und 56 Minuten für Einsatzfahrten ohne Sonderrechte auf Anfahrt.
In the course of the EUROPEAN PROJECT TRACE all fatally injured pedestrians autopsied at the Institute for Legal Medicine in Munich in 2004 had been analysed by using the "Human Functional Failure (HFF) analysis" method. It was possible to apply this method although some restrictions have to be taken into account. The results derived from this analysis comprise first the failures the pedestrians (most often "impairment of sensorimotor and cognitive abilities") and the opponents (most often " Non-detection in visibility constraints conditions") faced in the accident, second the conflicts and tasks (pedestrian crossing the street conflicting with a vehicle from the side (which was going ahead on a straight road), the degree of accident involvement (pedestrians often the primary active part), and further the contributing factors to the accident (pedestrians most often "alcohol (> 0.05% BAC)", opponents most often "visibility constraints").
Auf der Grundlage von 951 Unfallprotokollen aus den Jahren 1997 bis 2005, bei denen Bedienstete, Fahrzeuge oder Geräte der Straßenbauverwaltungen in Arbeitsstellen kürzerer Dauer (AkD) auf Autobahnen zu Schaden kamen, wurden relevante Unfallmuster herausgearbeitet. So konnte im Wesentlichen der Lkw als Hauptunfallverursacher mit einem Anteil von knapp 60 % am Gesamtunfallaufkommen identifiziert werden. Hauptsächlich sind AkD bei Sperrung des rechten Fahrstreifens oder des Standstreifens betroffen. Ein "Anprall von hinten" auf dem rechten Fahrstreifen und ein "seitliches Streifen" auf dem Standstreifen von zum Beispiel Absperrtafeln oder Fahrzeugen treten mit etwa gleichen Anteilen am häufigsten auf. Ferner zeigen die Auswertungen, dass 2/3 aller Unfälle bei stationären AkD aufgetreten sind. Besonders auffällig ist, dass fast 80 % der verunglückten Mitarbeiter in ihrem Fahrzeug zu beklagen sind, nur ca. 20 % der Verunglückten waren beim Unfall außerhalb der Fahrzeuge. Beim Ein- und Aussteigen sind nur 4 Unfälle dokumentiert, diese allerdings mit je zwei Getöteten und Verletzten. Hinsichtlich der relevanten Unfallmuster wurden Maßnahmen abgeleitet, die zum einen die Unfallvermeidung und zum anderen die Unfallfolgenminderung zum Ziel haben. Im Rahmen einer Abschätzung wurden diese Maßnahmen bewertet und das erwartete Reduzierungspotenzial, einerseits bezogen auf die Anzahl vermeidbarer Unfälle und andererseits auf die Vermeidung von Verletzten, abgeleitet. Die Maßnahmen zur Erhöhung der Sicherheit des Betriebsdienstpersonals in AkD wurden hinsichtlich ihrer Wirkungsweise (optisch, akustisch, haptisch) auf den Verkehrsteilnehmer bewertet. Aufgrund der haptischen Wirkungsweise auf die Verkehrsteilnehmer beim Überfahren von Warnschwellen könnte bei stationären AkD ein relativ großes Reduzierungspotenzial zu erreichen sein. Gleiches gilt auch beim Einsatz des Unfall-Vorwarn-Systems (UVS). Das größte Potenzial zur Vermeidung von Unfällen stellt die aktive Fahrzeugsicherheit bei den unfallverursachenden Verkehrsteilnehmern dar. Würden Lkw, die als Hauptunfallverursacher in Relation zu ihrer Fahrzeuganzahl deutlich überrepräsentiert sind, mit radarbasierten Fahrgeschwindigkeitsreglern und Spurassistenzsystemen ausgestattet, könnte mit relativ geringem Aufwand ein hohes Maß an Sicherheit erzielt werden. Nutznießer einer solchen Pflichtausstattung wären darüber hinaus natürlich auch alle anderen Verkehrsteilnehmer, die bei Auffahrunfällen etc. davon profitieren würden. Auf die Installation von aktiven Sicherheitssystemen in Fahrzeuge kann nur durch entsprechende Vorgaben des Gesetzgebers Einfluss genommen werden. Empfehlenswert erscheint außerdem der Einsatz von Unfalldatenschreibern (UDS) inklusive der Dokumentation des Fahrerverhaltens wie zum Beispiel Festhalten des Lenkrades, um den Unfallhergang im Nachhinein rekonstruieren zu können. Insgesamt bieten passive Schutzsysteme in den Betriebsdienstfahrzeugen wegen der hohen Anteile von Verunglückten in den Fahrzeugen das größte Potenzial zur Vermeidung von Verletzungen. So könnten bereits durch die generelle Empfehlung zum Anlegen des Sicherheitsgurtes, auch bei der Absicherung von Arbeitsstellen oder Kurzfahrten, rund 30 % der zu erwartenden verletzten Fahrzeuginsassen vermieden werden. Zur Unterstreichung dieser Maßnahme sollte in entsprechenden Sicherheitsunterweisungen auf die Unfallgefahr hingewiesen werden. Diese sollten nicht nur das jederzeit gegenwärtige Gefährdungsrisiko ins Bewusstsein rufen, sondern auch die Eigensicherung, wie zum Beispiel erhöhte Aufmerksamkeit beim Ein- und Aussteigen, das Anlegen der Sicherheitsgurte, die korrekte Sitzposition sowie die persönliche Kopfstützeneinstellung betreffen. Ein sehr großes Vermeidungspotenzial insbesondere von HWS-Verletzungen bieten zum Beispiel Nackenairbags, aber auch Kopfstützensysteme in Verbindung mit Gurtstraffern mit zu erwartenden Reduzierungen von über 80 % der Verunglückten in den Fahrzeugen.
While the number of fatal accidents is diminishing every year, there is still a need of improvement and action to prevent these deaths. Basis for this purpose has to be an analysis about the factors influencing the car crash mortality. There are various studies describing the univariate influence of several factors, but crash scenarios are too complex to be described by a single variable. The multivariate analysis respects the interference of the variables and gets so to more detailed and representative results. This multivariate analysis is based on about 2,600 cases (the data have been collected by the accident research units Hannover and Dresden (during the years 1999-2003). This paper presents a multivariate model (containing ten different variables) which detects 93% of these cases properly. This means it detects the cases as truly survived and truly death.
Bicyclists are minimally or unprotected road users. Their vulnerability results in a high injury risk despite their relatively low own speed. However, the actual injury situation of bicyclists has not been investigated very well so far. The purpose of this study was to analyze the actual injury situation of bicyclists in Germany to create a basis for effective preventive measures. Technical and medical data were prospectively collected shortly after the accident at the accident scenes and medical institutions providing care for the injured. Data of injured bicyclists from 1985 to 2003 were analyzed for the following parameters: collision opponent, collision type, collision speed (km/h), Abbreviated Injury Scale (AIS), Maximum AIS (MAIS), incidence of polytrauma (Injury Severity Score >16), incidence of death (death before end of first hospital stay). 4,264 injured bicyclists were included. 55% were male and 45% female. The age was grouped to preschool age in 0.9%, 6 to 12 years in 10.8%, 13 to 17 years in 10.4%, 18 to 64 years in 64.7%, and over 64 years in 13.2%. The MAIS was 1 in 78.8%, 2 in 17.0%, 3 in 3.0%, 4 in 0.6%, 5 in 0.4%, and 6 in 0.2%. The incidence of polytrauma was 0.9%, and the incidence of death was 0.5%. The incidence of injuries to different body regions was as follows: head, 47.8%; neck, 5.2%, thorax, 21%; upper extremities, 46.3%; abdomen, 5.8%; pelvis, 11.5%, lower extremities, 62.1%. The accident location was urban in 95.2%, and rural in 4.8%. The accidents happened during daylight in 82.4%, during night in 12.2%, and during dawn/dusk in 5.3%. The road situation was as follows: straight, 27.3%; bend, 3.0%; junction, 32.0%; crossing, 26.4%; gate, 5.9%; others, 5.4%. The collision opponents were cars in 65.8%, trucks in 7.2%, bicycles in 7.4%, standing objects in 8.8%, multiple objects in 4.3%, and others in 6.5%. The collision speed was grouped <31 in 77.9%, 31-50 in 4.9%, 51-70 in 3.7%, and >70 in 1.5%. The helmet use rate was 1.5%. 68% of the registered head injuries were located in the effective helmet protection area. In bicyclists, head and extremities are at high risk for injuries. The helmet use rate is unsatisfactorily low. Remarkably, two thirds of the head injuries could have been prevented by helmets. Accidents are concentrated to crossings, junctions and gates. A significant lower mean injury severity was observed in victims using separate bicycle lanes. These results do strongly support the extension or addition of bicycle lanes and their consequent use. However, the lanes are frequently interrupted at crossings and junctions. This emphasizes also the important endangering of bicyclists coming from crossings, junctions and gates, i.e. all situations in which contact of bicyclists to motorized vehicles is possible. Redesigning junctions and bicycle traffic lanes to minimize the possibility of this dangerous contact would be preventive measures. A more consequent helmet use and use and an extension of bicycle paths for a better separation of bicyclists and motorized vehicle would be simple but very effective preventive measures.
Injuries in motorbike accidents in correlation with protective clothes and mechanism of the accident

This study deals with a possible connection between safety clothing / accident mechanism and injury severity in a state-wide traffic accident investigation with focus on light and small motorbike-involvement for accidents in the area of the Saarland in which the persons riding the bike have been injured or killed. An interdisciplinary team of medical scientists and engineers collected the medical and technical data as well as all the relevant traces of the accident on scene and in time. During twenty months of data collection a total of 401 cases could be gathered. Grave injuries were more common for the group of heavier motorcycles (>125 ccm). Motorcyclists had been polytraumatized only in the group where the accident was connected with a collision. Significant correlation between protective clothes and injury severity could only be found for protective gloves and protective trousers. The knowledge about mechanism of the accident, protective clothes and severity of injuries can be helpful for the improvement of road and motorcyclists' safety.
Pedestrian accidents are one of the major concerns related with road accidents around the world. Portugal has one of the highest rates of pedestrian fatalities in Europe. In this paper an overview conditions were the pedestrian accidents occurred in Portugal is presented. In the last years, a project related with the pedestrian accidents has run in Portugal for the period 2004-2006 where 603 people died, 2097 have been severely injured and about 17000 slightly injured. Within this project all the pedestrian accidents in this period have been analysed providing global information about a wide range of aspects, since location, driver and pedestrian characteristics, weather and road conditions, among others. In addition, 50 in-depth accidents have been investigated and the data collected according the Pendant methodology. For this in-depth methodology detailed information about the accident has been collected, including injuries, vehicle damage, road conditions and road user- behaviour and actions. An accident reconstruction has been carried for each case including the determination of the speeds and driver actions, and the analysis of the contributing factors for the accident. Depending of the accident complexity, different methodologies have been used to analyse these accident, from the classical analytical equations such as Simms and Woods, to the use of detailed computational pedestrian models as those included in the commercial software- PC-Crash-® or Madymo-®. Also one of the goals of our investigation is the development of multibody models and methodologies for the reconstruction of pedestrian accidents. Some of these tools integrated in the commercial software Cosmos Motion-® are presented. The advantages of the different approaches are compared and discussed for some of the accidents investigated. With these tools the impact speed can be determined from the projection distance with analytical tools or PC-Crash-®, but more complex tools should be used to determine speed from the injuries, what is especially important for fatal accidents. The influence of the vehicle geometry and stiffness characteristics is another aspect analysed, where the influence of the vehicle stiffness has been determined using a combined multibody-finite elements approach within the software Madymo-®.
Nach wie vor ist die Anzahl von Unfällen motorisierter Einspurfahrzeuge (MESFz) mit sehr schweren Verletzungsfolgen oder tödlichem Ausgang für die Aufsassen im Vergleich zu allen anderen getöteten Verkehrsteilnehmern alarmierend hoch. Im Jahr 2013 wurden bei insgesamt 42.427 Unfällen MESFz 641 Aufsassen getötet und 12.034 schwer verletzt. Um dieser hohen Zahl schwerverletzter und getöteter Aufsassen von MESFz entgegenzuwirken, hat die Bundesanstalt für Straßenwesen (BASt) das vorliegende Projekt initiiert. Zielsetzung des Projektes war es, durch eine Analyse von Unfalldaten von MESFz, durch ergänzende computergestützte FEM-Simulationen und durch eine kritische Bewertung der UN-Regelung UN-R 22/05 (vormals ECE-R 22/05) hinsichtlich verletzungs-biomechanischer Inhalte (Versuche und Prüfwerte) Erkenntnisse zu erarbeiten, Handlungsbedarf festzustellen und Änderungs- und Ergänzungsvorschläge zur Überarbeitung der UN-R 22/05 sowie hinsichtlich allgemeiner Maßnahmen zu formulieren. Auf der Grundlage der amtlichen Unfalldaten des Statistischen Bundesamtes, Wiesbaden (DESTATIS) erfolgte eine Unfalldatenauswertung im Allgemeinen. Für eine detaillierte Analyse wurden Daten der "German In-Depth Accident Study" (GIDAS, Dresden und Hannover) ausgewählt. Bei etwa der Hälfte der im Teilumfang (n=199) untersuchten Unfälle von MESFz kam es zu Kopfverletzungen, überwiegend bei benutztem Schutzhelm. In 18 % lagen die führenden Verletzungen am Kopf; in 48 % blieb der Kopf unverletzt bei sonstigen schweren bis schwersten und tödlichen Verletzungen am Körper. Etwa 10 % der Aufsassen benutzten ein MESFz ohne bzw. mit absolut ungeeignetem Helm. Eine kritische Bewertung und Alternativvorschläge der derzeitigen Fassung der UN-R 22/05 wurden bezüglich der Punkte Prüfumfang, Prüfausstattung, Prüfdurchführung, Prüfkriterien und eine fälschungssicheren Homologations-Kennzeichnung erarbeitet.
Nationale Grenzen überschreitende Kooperation ist grundsätzlich auch im Bereich des Rettungsdienstes als sinnvoll und erstrebenswert anzusehen. Allerdings erfordert eine "alltägliche" Zusammenarbeit gerade im Bereich der Gefahrenabwehr und Daseinsvorsorge ein besonders hohes Maß an Abstimmung und Regelung, zumal die einzelnen Nationalstaaten über sehr unterschiedliche rettungsdienstliche Systeme verfügen. Dies gilt in besonderem Maß für den bodengebundenen Rettungsdienst, da dieser die reguläre rettungsdienstliche Versorgung sicherstellt. Die Luftrettung wird sowohl in Deutschland als auch in allen Anrainerstaaten " mit Ausnahme der Schweiz " immer als Ergänzung des bodengebundenen Rettungsdienstes durchgeführt. Mit dem Zusammenwachsen der Europäischen Union nimmt auch die Bedeutung grenzüberschreitenden Rettungsdienstes immer mehr zu. Die Bemühungen der Europäischen Union, die Zusammenarbeit der grenznahen Regionen (Euregios) zu fördern zeigen ebenso Wirkung wie die Aufhebung der Grenzkontrollen und die damit verbundenen politischen Entwicklungen. Die Abkommen der Bundesrepublik Deutschland mit allen Anrainerstaaten über die grenzüberschreitende Zusammenarbeit kommunaler Gebietskörperschaften ermöglichen es, auf lokaler Ebene bilaterale Kooperationsvereinbarungen zu schließen. Darüber hinaus bestehen seit zirka 20 Jahren ebenfalls Abkommen der Bundesrepublik Deutschland mit den Nachbarländern über die gegenseitige Hilfeleistung bei Katastrophen und schweren Unglücksfällen (Katastrophenschutzabkommen). Um der Komplexität des Untersuchungsgegenstands gerecht zu werden, erfolgte neben der Analyse der einschlägigen Literatur in einer explorativen Phase eine strukturierte schriftliche Befragung der einzelnen Bundesländer sowie eine teilstandardisierte schriftliche Befragung der relevanten Rettungsdienstbereiche. Abschließend wurde ein Experten-Workshop mit Vertretern aller beteiligten Organisationen durchgeführt. Als Problemfelder im Rahmen grenzüberschreitender Kooperation, die einer Regelung bedürfen, wurden die Bereiche Finanzierung/Kosten, Recht, Organisation, Kommunikation und Standards/Qualifikation identifiziert. Im Hinblick auf diese Hindernisse und Hemmschwellen gelang es, insbesondere auch durch den Erfahrungsaustausch im Rahmen des im Verlauf der Studie durchgeführten Workshops, hilfreiche Verfahrensweisen und Lösungsansätze aufzuzeigen. Durch die interdisziplinäre, alle relevanten Strukturen und Organisationen in der Bundesrepublik Deutschland umfassende Zusammensetzung der Teilnehmer des Workshops "grenzüberschreitender Rettungsdienst" und deren intensive und engagierte Bearbeitung der Materie entstanden im Ergebnis im gemeinsamen Konsens getragene - Prämissen und Kernpunkte eines Abkommens sowie Inhalte entsprechender vertraglicher Regelungen, - Rahmenrichtlinien zur Identifikation und Beschreibung qualitätssichernder Mindeststandards und Qualifikationsanforderungen, - Grundlagen und Vorgaben zur organisatorischen Ausgestaltung regelhafter grenzüberschreitender Kooperation. Aufgrund der heterogenen Rahmenbedingungen sowohl innerhalb der Bundesrepublik Deutschland als auch im Hinblick auf die über teilweise völlig unterschiedliche Systeme verfügenden Anrainerstaaten (insgesamt sind 10 Staaten in die Untersuchung einbezogen) bietet der erarbeitete Rahmen auch eine Grundlage für den Aufbau und die Ausgestaltung grenzüberschreitender rettungsdienstlicher Kooperationen in den nicht an die Bundesrepublik Deutschland grenzenden Regionen der Europäischen Union. Insgesamt stellen die Ergebnisse des Projekts somit neben der Beschreibung des derzeitigen Status Quo ein Kompendium und eine Handreichung zum Auf- und Ausbau sowie zur Optimierung der Ausgestaltung Staatsgrenzen übergreifender präklinischer notfallmedizinischer Kooperation dar. Der Originalbericht enthält als Anhänge unter anderm die in der Untersuchung eingesetzten Fragebogen und deren Auswertungen, Angaben zum durchgeführten Workshop sowie Beispiele zu den Themen Alarmdepesche, Symptomabfragekatalog gemeinschaftliche Alarm- und Ausrückordnung. Auf die Wiedergabe dieser Anhänge wurde in der vorliegenden Veröffentlichung verzichtet. Sie liegen bei der Bundesanstalt für Straßenwesen vor und sind dort einsehbar. [Introduction:] A large number of road users involved in road traffic crashes recover from their injuries, but some of them never recover fully and suffer from some kind of permanent disability. In addition to loss of life or reduced quality of life, road accidents carry many and diverse consequences to the survivors such as legal implications, economic burden, job absences, need of care from a third person, home and vehicle adaptations as well as psychological consequences. Within an EU funded project MOVE/C4/SUB/2011-294/SI2.628846 (REHABIL AID) these consequences were analyzed more detailed.
The objective of the study is to measure the risk of pedestrian and bicyclist in urban traffic through an analysis of real-world accident data. The kinematics and injury mechanisms for both pedestrian and bicyclists are investigated to find the correlation of injury risks with injury related parameters. For this purpose, firstly 338 cases are selected as a sample from an IVAC accident database based on the In-depth Investigation of Vehicle Accident in Changsha of China. A statistic measurement of the fatality and serious injury risks with respect to impact speed was carried out by logistic regression analysis. Secondly, 12 pedestrian and 12 bicyclist accidents were further selected for reconstruction with MADYMO program. A comparative analysis was conducted based on the results from accident analysis and computer reconstructions for the injury risk, head impact conditions and dynamic response of pedestrians and bicyclists. The results indicate that bicyclists suffered lower risks of severe injuries and fatalities compared with pedestrians. The risks of AIS 3+ injury and fatality are 50% for pedestrians at impact speeds of 53.2 km/h and 63.3 km/h, respectively, while that for bicyclists at 62.5 km/h and 71.1 km/h, respectively. The findings could have a contribution to get a better understanding of pedestrians" and bicyclists" exposures in urban traffic in China, and provide background knowledge to generate strategies for pedestrian protection.
The misuse of CRS (child restraint system) is one of the most urgent problems in connection of child safety in cars. Numerous field studies show that more than two thirds of all CRS are used in a wrong way. This misuse could lead to serious injuries for the children. Surprisingly the quality of CRS use is coded much better in accident data (e.g. GIDAS) than the results of observatory field studies show. It is expected that misuse of CRS was not detected by the accident teams in a large number of the cases. An essential part in improving child seats and their usability is the knowledge of the relation between misuse and resulting injuries. For that the analysis and experimental reconstruction of accidents is an important part. For allowing an exact experimental accident reconstruction, it is necessary to have detailed information about the securing situation of the child and about the installation of the CRS in the car.
Introduction: Spine injuries pose a considerable risk to life and quality of life. The total number of road deaths in developed countries has markedly decreased, e.g. in Germany from over 20000 in 1970 to less than 4000 in 2010, but little is known how this is reflected in the burden of spine fractures of motor vehicle users. In this study, we aimed to show the actual incidence of spine injuries among drivers and front passengers and elucidate possible dependencies between crash mechanisms and types of injuries.
To elucidate the risk of pedestrians, bicycle and motorbike users, data of two accident research units from 1999 to 2014 were analysed in regard to demographic data, collision details, preclinical and clinical data using SPSS. 14.295 injured vulnerable road users were included. 92 out of 3610 pedestrians ("P", 2.5%), 90 out of 8307 bicyclists ("B", 1.1%) and 115 out of 4094 motorcycle users ("M", 2.8%) were diagnosed with spinal fractures. Thoracic fractures were most frequent ahead of lumbar and cervical fractures. Car collisions were most frequent mechanism (68, 62 and 36%). MAIS was 3.8, 2.8 and 3.2 for P, B and A with ISS 32, 16 and 23. AIS-head was 2.2, 1.3 and 1.5). Vulnerable road users are at significant risk for spine fractures. These are often associated with severe additional injuries, e.g. the head and a very high overall trauma severity (polytrauma).
Although ATV accidents account for numerous deaths in the US and Australia, the role in traffic accidents and hospital admissions in Germany is unknown. At a level I trauma centre, hospital and crash charts were analysed for medical and technical parameters of ATV accidents. ATV drivers were 0.1% of emergency trauma patients. The mean total hospital stayrnwas 15 days; there were 1.5 stays per patients with 2.0 surgical procedures needed. One patient died, only two recovered fully. 14 cases of ATV accidents out of 18990 (0.1%) were documented within 10 years. The mean impact velocity was 35 km/h. Car collisions were predominant. The upper extremity was the predominant injured region (AIS 0.7), Mean maximum AIS was 1.4. ATV accidents in Germany are rare but pose high risk for severe injuries. Possible reasons are low active and passive security, limited experience and risky driving behaviour. Preventive measures are discussed.rn
This study aims to analyze spine injuries in motor vehicle accidents. Between 1985 and 2004 the Hannover accident research unit documented 18353 accidents. We identified 161 front passengers (0.53%) with cervical spine injuries, 84 (0.28%) with thoracic and 95 (0.31%) with lumbar injuries. Technical and medical data was reviewed. Patients" records were retrieved. X-rays were evaluated and fractures were classified according to the Magerl classification. 68% and 57% of thoracic and lumbar fractures occurred in accidents with multiple impacts. Delta-v was 50, 40 and 40 kph in passengers with cervical, thoracic and lumbar spine, resp. Passengers with spinal fractures frequently showed numerous concomitant injuries, e.g. additional vertebral fractures. The influence of seat belts and airbags is discussed. Patient work-up has to include a thorough investigation for additional injuries.
The purpose of this work is to investigate the association between the injuries in motorcycle accident and the main accident configurations. The data were provided by a multicentric case-control study MAIDS regarding the risk of crash and injuries of motorcyclists. Chi-square test was used to evaluate the relationship between the variables and a logistic regression was performed to evaluate the association of injury severity with some variables supposed to be predictive factors. Lesive patterns characterized by internal haemorrhages are mainly associated with fronto-lateral crashes, above all in urban areas. Lacerations or abrasions, mainly reported in torso and lower extremities, are mostly associated with single crashes or accidents in queue also for crashes occurred to low speed (< 50 km/h). The severity of injuries is highly associated with impact speed, regardless of the crash configuration. Fractures and haemorrhages play an important role in determining the severity of injuries. The upper extremities are the most frequently traumatised anatomic areas.
From an automotive safety occupant protection standpoint, effective occupant restraint requires a system capable of providing non-injurious occupant ride down of anticipated crash forces. This is not only the case for frontal collisions, where occupant restraint is provided primarily by seatbelts and airbags, but is also critical for other crash modes such as side impacts, rear impacts, rollovers, as well as multiple impact events. In the rear impact crash mode, occupant restraint is provided primarily by the seatbacks and to some extent the seatbelts. Foundationally, therefore, what becomes fundamental to the seatback's role in rear occupant protection is its ability to contain the occupant within the seat, preventing occupant ramping, as well as preventing the seat's, and/or its occupant's, dangerous intrusion into the rear occupant's survival space where contact with rear compartment components and/ or rear seated occupants can present a significant injury risk. An analysis is presented of a series of rear impact sled testing conducted by the authors that evaluates the timing, position and extent of the front seatback's reward displacement toward and into the rear occupant compartment as well as consideration of the front seat occupant' ramping potential and its injury potential relative to the rear compartment. Additionally, three other series of testing are presented which assess various seat designs occupant retention capabilities. Lastly, a matched-pair comparison test series is presented which evaluates occupant motion in rear impact with and without use of a typical vehicle body mounted 3-point seatbelt. Discussion of restraint system performance observed in all the testing is included along with ATD biofidelity and thigh-gap considerations. The data collected and presented includes accelerometer instrumentation and high speed video analysis.
Automotive interiors have long been a potentially injurious impact area to occupants during accidents, especially in the absence of adequate padding. The U.S. Federal Motor Vehicle Safety Standard (FMVSS) 201, Occupant Protection in Interior Impact, outlines test procedures and performance criteria in order to mitigate potentially injurious head impacts to interior surfaces. FMVSS 201 specifies a finite set of impact locations and applies to passenger vehicles of a specified year range and with a gross vehicle weight rating less than 10,000 lb. In this paper, two head impact test methodologies are presented, a pendulum-test device and a Free Motion Headform (FMH) launching device, which allows for dynamic, repeatable impact evaluation of various vehicle interior surfaces and their impact attenuation abilities. The presented testing includes multiple series that evaluate the effect of differing vehicle upper interior padding on occupant head injury. One study in particular, analyzes a head impact to the side header of a heavy truck (not included in FMVSS 201) during a 90 degree rollover. Additionally, two other series of tests are presented which assess the injury reduction effect of side airbags to near side as well as far side occupants in a side impact scenario. Lastly, a forensic analysis is presented which evaluates two possible head impact locations experienced in a real world accident by analysis of the resulting interior compartment damage utilizing the FMH launching device test method. The data collected and presented includes accelerometer instrumentation and high speed video analysis. These studies demonstrate that adequate padding and airbags are very effective at mitigating head injury potential at impact speeds of 12-25 mph (19-40 kph).
The following paper presents the nature and mechanism of injuries sustained in frontal impacts, focusing on car to car impacts. It was found that the body regions most frequently sustaining severe to fatal injuries were the legs and the thorax. The nature and mechanism of the injury sustained was investigated only for the thorax injuries, due to their potentially life threatening nature. The analysis revealed that the most frequent cause of the injury recorded was the seatbelt for low severity injuries and the front structure of the vehicle for higher severity injuries. An analysis of the effect of load limiter technology in the restraint system showed that the proportion of occupants who sustained "no thorax injury" did not increase when a load limiter was fitted to the restraint system. However, a decrease in the "organ" and "organ and skeletal" injuries was observed in the load limiter sample. Sample size and variation mean that these findings are not conclusive.
Die Bundesanstalt für Straßenwesen (BASt) bringt zum Ende jeden Jahres eine Prognose der Unfall- und Verunglücktenzahlen des noch laufenden Jahres heraus, um so über die Entwicklung der Verkehrssicherheit in Deutschland Bilanz ziehen zu können. Dabei wird das Unfallgeschehen nach dem Schweregrad der Konsequenzen, der Ortslage sowie Alter und Art der Verkehrsbeteiligung der Verunglückten in 27 Zeitreihen unterteilt. Zu diesem Zeitpunkt sind die Daten lediglich für die ersten acht oder neun Monate erhältlich. Um Bilanz zu ziehen, werden die Anzahlen der letzten drei oder vier Monate prognostiziert. Gesamtziel des hier beschriebenen Forschungsvorhabens ist die Optimierung der jährlichen Unfallprognosen durch Anwendung von strukturellen Zeitreihenmodellen, bei denen die Vorhersagen aus dem Trend der vorliegenden Monate, und der Dynamik der vorhergehenden Jahre abgeleitet werden. Um dem Einfluss der Witterungsverhältnisse Rechnung zu tragen, werden dabei meteorologische Variablen in das Vorhersagemodell aufgenommen. Um die Modelle zu testen, werden die endgültigen Daten der letzten 15 Jahre jeweils aus den vorläufigen Daten der ersten Monate vorhergesagt und mit den tatsächlich beobachteten endgültigen Unfall- und Verunglücktenzahlen verglichen. Die Resultate zeigen, dass im Vergleich zu den bisherigen Vorhersagen mithilfe der hier vorgestellten Modelle die Vorhersagen für 25 der 27 Reihen präziser werden. Lediglich zwei Reihen zeigen einen leichten Anstieg des Vorhersagefehlers. Beim Vergleich von Modellen mit und ohne meteorologischen Variablen zeigt sich, dass 23 der 27 Reihen besser vorhergesagt werden können, wenn man das Wetter berücksichtigt. Neben der verbesserten Vorhersage ermöglicht die Aufnahme der Wettervariablen auch eine Einschätzung, wie groß der Einfluss der Witterungsgegebenheiten auf das Unfallgeschehen ist. Es zeigt sich also, dass die Anwendung von strukturellen Zeitreihenmodellen und die Berücksichtigung von meteorologischen Variablen zu einer deutlichen Verbesserung der Vorhersagegenauigkeit führen. Die Verbesserung der Vorhersagen durch die Aufnahme von Wettervariablen bestätigt nochmals den Einfluss der Witterungsumstände auf das Unfallgeschehen.
While many medical studies have dealt with the incidence, nature and treatment of polytrauma the injury-causing accident mechanisms are rarely discussed in detail, mostly due to the lack of documentation of the technical aspects. The present prospective study was started in late 2007 and collects data from traffic accidents with most severely injured in six south- German counties and two larger cities for the duration of one year. It is aimed at identifying and documenting all polytrauma cases (ISS ≥ 16) caused by traffic accidents and their crash circumstances. The data collection is based on an interdisciplinary concept to include both the police, emergency dispatch centers, hospitals and fire departments in the region and is completely anonymous. Potentially relevant cases where an emergency physician was called to the scene of a traffic accident are provided by the dispatch center. All three hospitals in the region suited for the treatment of polytraumatised patients record injuries, major diagnostic and surgery data. Data and images from the accident scene are provided by the police and by fire departments. The latter provide information which is usually not available from the police, like deployed airbags, vehicle extrication measures and detailed views of car interiors. The main objective of the study is to determine the structure of road users who sustain a polytrauma, their crash opponents and the injury patterns found in relation to the collision configuration and the protection by seat belts, air bags and other devices. With detailed documentation of vehicle damage and extrication measures the study is also intended to support the development of injury predictors for pre-hospital treatment and provide field data regarding further improvement of technical rescue.
A total survey of road traffic accidents involving most severely injured, defined as sustaining a polytrauma or severe monotrauma (ISS > 15) or being killed, was conducted over 14 months in a large study region in Germany. Data on injuries, pre-clinical and clinical care, crash circumstances and vehicle damage were obtained both prospectively and retrospectively from trauma centers, dispatch centers, police and fire departments. 149 patients with a polytrauma and eight with a severe monotrauma were recorded altogether. 22 patients died in hospital. Another 76 victims had deceased at the accident scene. In 2008, 49 % of patients treated with life-threatening injuries were car or van occupants, 21 % motorcyclists, 18 % cyclists and 10 % pedestrians. Among fatalities at the scene, vehicle occupants constituted an even larger portion. The number of road users with life-threatening trauma in the region was extrapolated to the German situation. It suggests that 10 % among the "seriously injured" as defined in national accident statistics are surviving accident victims with a polytrauma or severe monotrauma.
Pelvic fracture, cracking or breaking of a portion of the pelvis are extremely common injuries in the side impact collisions of motor vehicles. Due to both its shape and structural architecture, mechanics of the pelvic bone is complicated. There is a lack of knowledge regarding the dynamic behavior of the pelvis and its biomechanical tolerance under impact environment. Hence this study is aimed at the understanding of the mechanical response of the human pelvis with three-dimensional finite element (FE) models, under side impact load, applied through a structure, equivalent to a car door. The door structure was modeled, considering few layers, consisting of foam (Styrodur®, 3035 CS), plastic (UHMWPE), steel, glass and steel, putting them in series. A soft tissue layer (equivalent to fat) was also considered on the greater trochanter location. These FE models (with and without the car door structure) were analyzed with ANSYS-LS-DYNA-® dynamic finite element software to compare the effect of the car door padding system for shock absorption. It was observed that with proper combination of shock absorbing material (foam, etc.) and its thickness, the transmission of impact load to the body part (pelvis, etc.) from the outer surface of the car door could be reduced.
This study aimed at developing an injury estimation algorithm for AACN technologies for Germany and compared them to findings based on Japanese data. The data to build and to verify the algorithm was obtained from the German in-depth Accident Database (GIDAS) and split into a training and a validation dataset. Significant input variables and the generalized linear regression model to predict severe injuries (ISS>15) were selected to maximize area under the receiver operating characteristic curve (AUC). Probit regression with the input parameter multiple impact, delta v, seatbelt use and impact direction gave the largest AUC of 0.91. Sensitivity of the algorithm was validated at 90% and specificity at 76% for an injury risk threshold of 2%. It appears that no major differences between Japan and Germany exist for injury estimation based on delta v and impact direction. However, far side impact and multiple crash events appear to be associated with a larger risk increase in the German data.
